Further support services include pharmacy services, clinical pharmacy, case management, chronic disease management, eligibility assistance, health education, outreach, translation, environmental health services, nutrition, and telehealth services. Gracelight strives to ensure that even the most vulnerable populations have equitable access to high-quality healthcare, which will help improve overall community health and well-being in the Los Angeles area.
