Governance

This organization files the short tax form, which does not ask about boards, policies or audits. That is normal for a small charity and is not a mark against it.

Impact evidence

Describes programs. Its tax return describes what it does but we have found no measurable results in it.

Programs described in its return (1)
  1. $68,500 spent. Since inception, CICH has provided training to over 100 certified Community Health Workers (CHW) to address the physical, emotional, and social needs of individuals. Throughout the program, CICH strives to create valuable and knowledgeable members who will be able to: * Identify social determinants that are present in the communities. * Address low health literacy and other barriers that affect health care. * Fill the gap between primary care and public health. * Utilize prevention strategies to enhance overall health. * Empower individuals to achieve better health. * Reduce health disparities. The ultimate goal of this project is to deploy these trained CHW residents back into the community to directly help their neighbors improve their health and well-being (Prevalence of diabetes and chronic diseases, etc.).by raising personal health awareness through informing, educating and supporting them. Specifically, we want to create a sustainable model led by Certified CHW's to introduce and facilitate diabetes and chronic disease self-management workshops in neighborhoods where there is a heavy burden of diabetes and chronic conditions.
