Programs described in its return (1)
- $175,585 spent. Foundational Community Supports (FCS) Pathfinder Program: Provided Permanent Supportive Housing (PSH) and Individual Placement and Support (IPS) services to approximately 300 individuals experiencing homelessness across King, Snohomish, and Pierce counties in Washington State. Services were delivered through the Pathfinder case management model, which employs staff with lived experience of homelessness to serve as peer navigators. Pathfinders met clients where they were, including in unsheltered settings, and provided housing navigation, employment support, benefits enrollment, and community integration services. The organization operated under a by-and-for philosophy in which the people closest to the problem are recognized as experts in solving it. Services were funded through Washington State Medicaid Waiver (Foundational Community Supports) billing administered by Wellpoint Washington. The organization also distributed $18,526 in Transitional Assistance Program (TAP) funds directly to individual clients to address immediate barriers to housing stability, including move-in costs, essential supplies, identification documents, and other needs as identified by each client and their Pathfinder.
