EACH PARTICIPANT IN THIS PROGRAM COLLABORATES WITH THEIR ESTABLISHED SUPPORT TEAM, A UNIQUE PERSON-CENTERED PLAN BUILT TO HELP MEET INDIVIDUAL DESIRED OUTCOMES AND NEEDS.
