- Jefferson's unique governance structure continues as a combined board with equal representation from jefferson, abington, jefferson health - northeast and the former kennedy health system [jefferson new jersey]. During fy22, jefferson health and einstein healthcare network merged in october 2021. Jefferson health added lehigh valley health network in august 2024. This expansion of jefferson health creates an integrated 32hospital health system throughout eastern pennsylvania and southern new jersey focused on providing greater access to high-quality patient care in our communities and delivering outstanding health sciences education to tomorrow's health care professionals. In keeping with lh's commitment to its community members, the following services were provided to the community, particularly to low income members within its service area, during the year: - emergency department (operates 24 hours a day/365 days a year) open to all members of the community regardless of their ability to pay. Ah is the umbrella organization that encompasses its flagship hospital, amh, in abington and lh in hatfield township. Together, amh and lh have 807 licensed beds, seven outpatient campuses: abington health-blue bell, abington health-elkins park, abington health-lower gwynedd, abington health-montgomeryville, abington health-warminster, and abington health-willow grove and abington health center-horsham. There is one urgent care center in flourtown, pennsylvania. Physicians employed practices are located in bucks, montgomery and philadelphia counties with 74 locations. The above branded, fictitious names are used for merger announcement purposes. Legal names of amh, ah, ahf and lh will be used. Lh offers a sophisticated array of clinical programs, including: interventional radiology, orthopaedics, physical rehabilitation, outpatient surgery, cardiology, ophthalmology, podiatry, oral surgery and 24-hour emergency services. Lh is located just outside of lansdale, pennsylvania, approximately 24 miles northwest of philadelphia. Nestled in the heart of montgomery county, lansdale is one of the six boroughs that make up the area known as north penn, one of the region's most prosperous and desirable locations. According to recent demographic information, lansdale is home to an estimated 51,000 residents. However, lh serves all of the north penn communities, as well as parts of neighboring bucks county, for a total patient base nearing 200,000. From small local enterprises to large multi-national corporations, business is booming in the north penn area. Montgomery county has the highest per capita income in the state of pennsylvania, and the second highest median income. Lansdale is served by the north penn school district, with thirteen elementary schools, three middle schools and one high school. The area is also home to numerous private and parochial schools, technical career centers, four-year colleges and two-year colleges. The lansdale/north penn area offers an exceptional quality of life, with quaint suburban towns, quality schools, a healthy economy, good jobs, abundant recreation and convenient access to philadelphia, new york, the poconos and the new jersey shore. Lh is guided by the belief that it is dedicated to the healthcare needs of the communities that it serves. That level of determination and commitment is the cornerstone of lh. Moreover, lh, under its financial assistance policy, provides services without charge or at amounts less than its established rates to patients who are unable to compensate lh for their treatments either through third party coverage or their own resources. Since charity care amounts are not expected to be paid, they are not reported as revenue. In addition, lh provides services and supplies at below cost to persons covered by government programs, including medicare and medicaid. Coordination continued this year, centralizing its financial counselors from within the system into the financial counseling program and expanded staff to lh. In addition, a social worker supports the dental access program. A financial counselor helps determine eligibility for government-sponsored programs and offers assistance with the health insurance exchange and other insurance coverage. If an individual does not qualify for other coverage, they assist individuals and families in the completion of the jefferson health and ah financial assistance application. Community resources are shared with lh staff to support patients and families as well as including county and area non-profit providers on hospital programs and services. Lh has bolstered its medical staff, adding top specialists in areas that include emergency medicine, neurosurgery, orthopaedics, spine surgery, radiology, anesthesia, pathology, surgical and in 2019 included the addition of endocrinology, urology, neurology and full time pulmonary and intensive care specialist coverage. Lh has also expanded its primary care practices. Lh introduced a robotic surgery program in fy22 and now offers the option of minimally invasive robotic surgery for hernia repair, gallbladder removal or other general surgery procedures, as well as robotic surgery for knee replacements. The hospital added two distinct robotic platforms to perform orthopedic and general surgeries. Lh became an age friendly hospital to meet the needs of the community's demographics which boast a large senior population. The jaisohn medical center opened on the lansdale campus in 2020 and the relationship continued with lh in fy22. The center is located in the community health center building, on the campus of lh, which is home to the jaisohn medical center providing medical, social and educational services for korean americans and others. The center was named after dr. Philip jaisohn, the first korean to obtain a medical degree in the united states. Jefferson enterprise -------------------- Mission: we improve lives. - In october 2001, the hospital board negotiated the sale of north penn hospital to universal health services of king of prussia, pennsylvania. The transaction provided financial resources to improve capital equipment and expand services on the hospital's campus. As an important part of the consideration for the purchase of the hospital, the parties agreed to preserve the charitable assets that had been accumulated by the hospital over four decades by establishing the north penn community health foundation [now known as healthspark] whose mission is to identify, select and invest in programs and agencies that will improve the health, welfare and quality of life of its community. Since the acquisition of lh under the ah umbrella in november 2008, administrators, medical staff members and board members have worked to strengthen relationships with other agencies and providers including the healthspark foundation (former north penn community health foundation), the bucks-mont collaborative, the north penn school district, local ems squads, and other community-based organizations. Regionally, lh began a collaboration with other hospitals and health systems to form coach: "collaborative opportunities to advance community health" in 2016 and has continued working with the health care improvement foundation and the hospital and health system association of pennsylvania. Public health experts and hospital leaders collaborated on regional and local community needs and began a dialogue to focus on health improvement. Initiatives included a focus of food insecurity, the community health needs assessment process, and renewed efforts of centralizing and communicating community resources. Lh continued to increase community engagement activities by fulfilling requests and invitations from collaborative partners and provided outreach calls during the covid-19 pandemic. Compilation of community resources increased and were posted and shared with collaborative partners, community groups, stakeholders and faith communities. Lh leaders continued in the participation and leadership of the bucks-mont collaborative and invited community health staff to participate. This collaborative includes the mission to foster information sharing, dialogue, and collaborative action around health and wellness issues in montgomery and bucks counties to strengthen non-profit organizations through monthly membership meetings that provide timely information and updates including pandemic, economic and safety net initiatives, networking with agency, business, education, faith, and government leaders; and provides leadership training and development. Since its founding, the bucks-mont collaborative for health and human services has worked through its board of directors to uphold and promote the provision of high-quality services to all constituencies. Ah leaders serve actively on the board and continue to link and connect lh to the local non-profit organizations and key community stakeholders. Hospital leaders also focus efforts with the montgomery county office of public health, drug and alcohol, public safety and other departments. As a member of ah, lh, also a 501(c)(3) tax-exempt organization, offers a generous financial assistance policy to ensure free or low-cost services to patients who do not have insurance. - Community support, activities and programs ========================================== Outlined below are a number of lh community benefit programs. The information is not intended to be all-inclusive but rather provides additional information that further demonstrates how lh benefits the surrounding community in furthering its charitable tax-exempt purposes. - sponsorship of community organizations serving the health and social service needs of the lh community. - lh walking trail - an onsite walking trail historically used by local neighbors. Lh continues to maintain this walking trail each year for our local community. - volunteers: lh's volunteer services department coordinated community volunteers to serve the healthcare needs of their neighbors. Jefferson lansdale hospital provided 19,068 volunteer hours to the community. Lh, in addition to those outlined above, offers numerous other community programs and activities including, but not limited to the following: - information and referral to virtual support groups - american red cross blood drives - cpr training center (american heart association cpr training center) - flu immunization program - faith community nurse network - outreach efforts including follow up calls for hypertension and chronic care were offered to our community. Community health services ------------------------- Ah has dedicated itself to the community by providing a department responsible for the community health needs assessments, implementation plans, community benefit initiatives, health education, screenings, faith community network, cpr training center, and a chronic disease management program. In addition, ah provides clinics to serve those in need including primary care and specialty clinics geographically located within the service area; space for support and self-help groups, though suspended in fy22 due to the pandemic, a process was created for future re-engagement and onboarding. During the pandemic, lh outreach provided a pivot to follow up calls, creating educational fact sheets for community services, videos, social media education and mental health and substance use disorder resources. Jefferson lansdale hospital and vmsc emergency medical services launched a one-year pilot program aimed at reducing the number of patients with congestive heart failure and chronic obstructive pulmonary disease being re-admitted to the hospital within 30 days of discharge. Consenting patients get a home visit from a board-certified community paramedic within 24 hours of discharge and receive up to three additional home visits within the first 30 days post-discharge. Vmsc and jefferson health launch the region's first pre-hospital whole blood transfusion program This initiative represents a major step forward in emergency medical care for the north penn and indian valley communities. The ability to administer whole blood transfusions before a patient even reaches hospital could mean the difference between life and death for patients in critical condition. Until now, paramedics could only rely on iv fluids, which help maintain blood pressure but do not restore lost blood volume or oxygen-carrying capacity-key factors in patient survival. According to the journal of trauma and acute care surgery, pre-hospital whole blood transfusion has been shown to reduce hemorrhagic shock mortality by nearly 30 percent. This means the survival rate of trauma patients will be four times higher than those receiving iv fluids alone. Less than one percent of emergency agencies nationwide can administer whole blood transfusions before hospital arrival. Jefferson health is proud to partner with vmsc emergency services to provide this life-saving program to our north penn and indian valley residents. Impact of prior community health needs assessment and implementation Jefferson Lansdale hospital develops targeted health outreach programs and screenings in response to the identified needs of our community in concert with the mission of jefferson health: we improve lives. Lh works to create the healthiest community by orchestrating targeted outreach for maximum community benefit, while reducing health disparities. In fiscal year 2025, jefferson lansdale hospital provided members of our communities seeking resources for care and education, in alignment with our community health implementation plan developed in response to our 2025 community health needs assessment: - Support was provided to the community in uninsured and underinsured health services, including adult day services program, and dental care access program. In addition, the children's clinic screens all families for food insecurity and provides opportunities for these families to access fresh fruits and vegetables through a partnership program with montgomery county office of public health. - jefferson lansdale hospital provides many free or low- cost programs throughout the year designed to educate the community regarding health risk factors, chronic disease prevention or to support early detection through health screenings. In addition to cardiovascular-related health screenings, a free cancer screening day is held twice a year. jefferson lansdale hospital collaborates with many community organizations to support community health improvement initiatives: - jah safe center - jlh age-friendly care award / distinction - jlh warm handoff program for mental health - 988 campaign (national suicide hotline awareness) - integrated social worker/ care coordinator for mental health services into jefferson owned primary care physician offices - provided mhfa (mental health first aid education) for community (partnered with mcoph) - telehealth option to increase access to care - primary stroke center designation by the joint commission - acute heart attack ready designation by the joint commission - women's center of montgomery county on site with medical advocate and virtual sexual assault nurse examiner (sane) - jaisohn center on site p
