Programs described in its return (1)
- $391,580 spent. The organization primarily assists chronic care patients who suffer from hypertension, heart disease, diabetes, and other forms of chronic ailments. In 2024, 651 patients were seen for 2,100 appointments. New patient growth has doubled since 2022 with 254 new patients in 2024. All patients are uninsured and 80% are in the $0-$29k household income range. We had 10 volunteer providers and 2 volunteer pharmacists. We provided 895 prescription drugs free of charge to our patients and 129 patients were seen by two providers at our partners clinic, Granbury Eye Clinic. Our food pantry provided food for an average of 396 individuals per month with an average of 24 new families registering to receive food each month. We distributed 66,805 pounds of food in 2024. We had 24 new volunteers commit to unloading, sorting, packing, and distributing food. Our volunteers served 1,743 hours, with an average of 14 volunteers per month. The outreach center provided after school reading help for 8 children in the Oak Trail Shores community. In 2024 we implemented three new programs, Parent Cafe, Teen Mental Health group, and Oak Trail Connections. Each of these programs targets a different demographic and provides access to education, meals, fellowship, information about available services, and even opportunities to earn an income! Parent Cafe (in partnership with Cook Childrens) attendance ranged from 4-7 parent participants once per month and 9-23 children were provided free dinner and care while the parents participated in the Cafes. Teen Night attendance ranged from 1-11 teen participants, who also received a meal!
