- $2.7 million spent. Indonesian Basin-Since 2007, Health In Harmony has collaborated with Indonesian partner ASRI and communities around Gunung Palung National Park to create and implement community-designed programs that support human health, alternative livelihoods, and environmental conservation. HIH, ASRI and the Indonesian Ministry of Health are working together to design a way to scale the model of investment in community-designed solutions and provide last mile health care to remote rainforest communities across Indonesia. Priority sites have been identified across Papua, Kalimantan, and Sulawesi for phased implementation from 2026 to 2028. These locations have been selected for their remoteness and their proximity to areas of significant ecological importance, setting the stage for expanded delivery of mobile clinics, last-mile healthcare, and integrated conservation investments.
- $2.3 million spent. MADAGASCAR In fall 2019, HIH began working alongside communities around Manombo rainforest to realize their solutions to protect and restore the rainforest including, programs to address food security, alternative livelihoods, reforestation, and education. This protected area in Southeast Madagascar is home to nine endangered lemur species, as well as other threatened species. Manombo has been the stage for groundbreaking research in community-led, small-scale black soldier fly (BSF) farming since 2024, with outstanding results in terms of impact on one of the main reasons of deforestation in Madagascar hunger. In 2023, HIH signed a partnership agreement with Mdecins Sans Frontires / Doctors Without Borders (MSF) and Malagasy NGO Ny Tanintsika (NT) to begin an interdisciplinary pilot project in Ikongo, Madagascar. This new partnership combines HIH's Radical Listening and Planetary Health approach with MSF's medical expertise and NT's local systems and practice knowledge to protect and expand Madagascar's Ambositra-Vondrozo Forest Corridor and improve the health and socio-economic well-being of people. Expanding upon this partnership, Health In Harmony and its local affiliate launched an Ikongo-based chapter of the successful forest guardians program that can complement other initiatives such as building schools, promoting sustainable farming, primary care for young children, and Radical Listening all possible through this network of partners. The highlight of 2025, however, was the official incorporation of HIH Madagascar into Fahasalamana Mirindra, or FAMI, which allows staff more freedom to operate locally and respond to Madagascar's context as a native organization. DEMOCRATIC REPUBLIC OF THE CONGO In 2025, Health In Harmony officially began partnerships in the Democratic Republic of the Congo with local NGO Strong Roots Congo. This collaboration supports community-led conservation efforts across two forest concessions: Asu'u and Tubeumoyi, securing legal forest tenure over 54,738 hectares of critical Congo Basin rainforest. These efforts support an estimated 71,599 people while advancing community-designed solutions for forest protection. Most organizations working in the DRC focus on securing much-needed land tenure rights together, Health In Harmony and its partners step into a different niche, working with communities who are already managing territories and now need to make sure their climate solutions for the DRC's rainforests and its biodiversity are fully resourced.
- $1.4 million spent. BRAZIL After building a successful track record of deploying health expeditions, supporting sustainable forest economies, and advocating for community rights in the Xingu Basin since 2020, SAMA Health In Harmony officially achieved legal incorporation as Brazil's first Planetary Health entity in November 2024. Building on this foundation, the organization is advancing its Planetary Health model by directly supporting community-designed conservation strategies within extractive reserves (RESEX Rio Iriri, RESEX Rio Xingu, and RESEX Riozinho do Anfrsio) and scaling through the People's Climate Coalition across nine unique Indigenous Territories: TI Apyterewa, TI Cachoeira Seca, TI Araribia, TI Tabalascada, TI Malacacheta, TI Canauanim, TI Urubu Branco, TI Saraua, and TI Xipaya. By merging ancestral wisdom with modern monitoring tools like drones and GPS, these Indigenous and traditional communities are leading their own territorial defense and driving exceptional returns on investment through averted carbon emissions. The total area protected by these combined initiatives is over 3.7 million hectares. Xingu was also at the forefront of advocacy in 2025 as COP30 took place in Belm, Par and Health In Harmony and SAMA were honored to join many advocates from all over the world with a focus on Indigenous, Afrodescendant, and local communities in breaking silos to address the climate crisis through those who have had the solutions all along. Highlights supported by HIH and SAMA included territory protection expeditions, vaccination expeditions, malaria research, and commissioning a film in the Xipaya Indigenous Territory to show the vision of female leaders of the Xipaya Indigenous Peoples for the planet. PANAMA In 2025, Health In Harmony continued implementation across four sites in Panama's Darin region, building on work initiated in 2024. The focus of this effort is on robust Monitoring, Reporting, and Verification (MRV), including tracking carbon, ecoacoustics, and the Community Thriving Narrative (CTN). This work supports four communities, Alto Playn, Ember Pur, Arimae, and Puerto Lara, by strengthening data-driven insights into ecosystem health and community well-being. Nearly 2,500 Indigenous communities are protecting tropical rainforest while addressing challenges of regional migration, and Health In Harmony's role includes partnering with Indigenous-led environmental organization Savimbo to monitor local biodiversity; community members were trained in non-invasive monitoring methods such as bioacoustics and camera trapping, which identified 70+ species, of which 20 are considered ecological indicators and 13 are threatened. Following community requests, Health In Harmony has also assisted them in establishing necessary groundwork to begin selling biodiversity credits through Savimbo, launching with 47,125 pre-certified credits that directly finance the guardians of the Darin rainforest. HIH aims to support the communities in full credit certification, tripling each credit's value, throughout 2026. Panama was also the first location to showcase its Community Thriving Narratives (CTN) videos: led by HIH in partnership with If Not Us Then Who? (INUTW) and FSC-Indigenous Foundation, CTN fosters community members in creating stories rooted in their lived experiences, cultural knowledge, and vision for the future of their forest and territory. Local protests during film production were recorded with professional-grade equipment and skill, showing the world a reality Indigenous communities felt unseen without their own voices' impact. With the communities' content they, along with HIH and its partners, launched a global alert campaign aimed to support those affected by police brutality during the protests; thousands of dollars were raised in weeks to support mental health for youth, lawyer fees, and more.
