  1. $5.5 million spent. Envisage is a long-term planning solution for adults age 60+ that allows them to anticipate their future care needs. Plan Members are able to receive supportive services and care in their own homes for as long as safely and medically practical. After a Member's eligibility is determined, having executed the program documents, the Member will be visited by a Personal Wellness & Care Coordinator. The Personal Wellness & Care Coordination Team will make regular visits at least annually to Members to identify any changes in health and functional status. When changes occur, arrangements will be made and, when covered by the Plan, will be provided at no cost to the Member according to the Plan options chosen at enrollment. Envisage now offers a "Signature" plan, which provides only care coordination services. Envisage was purposeful in the implementation of these plans with a goal to expand the target market to individuals that may have lower asset base or lower monthly income.
